INSURANCE & PAYMENTS
NOTIFICATION: OUT OF NETWORK on certain Insurance Plans
We do not accept BLUE SELECT, or My Blue.
We are not on certain Humana Plans.
As of November 1, 2026 we will no longer be contracted with Aetna Medicare Advantage Network by their notification.
We are not on some United Healthcare Medicare Advantage plans.
Verify if your plan allows you to see your provider OUT of Network and what your deductible is for this option.
Insurance:
We accept nearly all major insurance plans. Please contact our office to verify if we accept your insurance.
There are some supplemental plans offered by insurance carriers today that we may not accept.
Additional Fees:
There is an additional $20 dollar fee for completion of forms for use outside of the patient's medical record, including:

No Show Fee:
There will be a $25 dollar no show fee charged to your account, if you do not show up on the day of your scheduled appointment without notice.
To avoid this fee please contact our office at least 24 hours prior to your appointment time, so we may conveniently reschedule you and contact another patient on our waiting list.

Insufficient Fund Check Fees:
There will be a $40 dollar check fee charged if a check written to our practice is returned by the bank due to insufficient funds.
We are charged fees by our bank when a check does not clear, therefore we must pass this fee onto you to be paid in addition to the amount originally billed for services.
